Prior authorization, or PA, is your insurance plan's way of approving a medication before it agrees to pay. The plan wants to confirm the drug fits its coverage rules for your diagnosis. It isn't a denial. It's a checkpoint.
What the insurer usually wants
Requirements differ by plan, but requests tend to include:
• Your diagnosis and relevant medical history
• Recent lab work, where it applies
• Other treatments you've tried and how they went
• Your prescriber's notes on why this medication is appropriate
Some plans also use step therapy, which means trying a lower-cost option first.

If it's denied
A denial is not the end. Your plan has to tell you why, and you can appeal. The fix is often simple: an extra chart note, a corrected diagnosis code, or proof of a previous treatment. Your provider can also talk through alternatives while an appeal is pending.
What the drug is approved for
Mounjaro (tirzepatide) is FDA-approved for type 2 diabetes. Zepbound contains the same active ingredient and is approved for chronic weight management.
